In Chapter 8, near the beginning, in your new home, when you follow Hugo down the stairs to Lucas' lab keep going all the way down the stairs and explore the crypt at the bottom. Interact with the tomb in the middle of the room to get the achievement.

In Chapter 4 when Hugo runs off he actually wants to play hide and seek - he's not just being a pain! Sneak up on him and find him before he finds you to unlock this achievement.

Enter through the main door

In Chapter 9, when you reach the university, don't take the easy route in through the side door - instead take out the two guards and enter using the main door behind them. This is just after the big semi-circular open area where you can use a rock to have a suspended crate of rats drop and break to kill some other guards.
