Prior to 2.0, many achievements were missable - but this is no longer the case as 2.0 added full respec options so your character is no longer locked into specific skills choices.
The few missable achievements remaining are the mutually-exclusive endings achievements, one collectible achievement (Breathtaking), one missable objective (
The Devil), and the achievement for taking lifepath-specific conversation choices (
Stanislavski's Method).
Both the base game and the Phantom Liberty DLC have multiple endings with mutually-exclusive achievements for each. Be sure to make a hard save at the point of no return for each storyline!
If you have the Phantom Liberty DLC, the optimal path is to complete the base game all the way up to the point of no return - then go complete Phantom Liberty (all endings) and then return to the base game to finish that off.
For the base game, hard save before you meet Hanako at Embers. This will be burnt into your brain by the time you get to this part of the story. So once that's your objective, stop, save, complete everything else you want to in Night City (and do the DLC if you have it).
For Phantom Liberty, make your hard save in the 10th main quest ("Firestarter"), when you are in the lab with Songbird - as you'll then need to make various choices from that point on to experience each of the endings. See the individual achievements for each.

Autojock
RareBuy all vehicles available for purchase.
Since more vehicles were added to the game over time this is no longer all vehicles, just quite a lot of them. By late game you should have quite a lot of cash - use it up to buy up all the vehicles you can, cheapest first and in ascending price order to maximise how many vehicles you can buy. It'll pop after a bunch have been bought. Keep going back to the kiosks to buy more until you get there.
Breathtaking
RareCollect all items that once belonged to Johnny Silverhand.
Bushido and Chill
UncommonWatch Bushido X with Rogue.
Christmas Tree Attack
CommonComplete a Breach Protocol with a minimum of 3 daemons uploaded.
City Lights
RareComplete all gigs and NCPD Scanner Hustles in City Center.
Daemon In The Shell
LegendaryKill or incapacitate 3 enemies with one "Detonate Grenade" quickhack.
Dirty Deeds
RareComplete every Gig in Dogtown.
There are nine gigs in Dogtown, and they'll appear on your map automatically. Not all are visible immediately, but will appear as you progress through the Phantom Liberty story.
Easy Come, Easy Go
RareSteal the Arasaka medical truck or deliver 10 vehicles to El Capitan.
Frequent Flyer
RareFind all fast travel dataterms.
Full Body Conversion
CommonInstall at least one implant in each system and body part.
Whenever you visit a ripperdoc you'll see a map of your body with all the slots shown - just install something in each one. By mid-late game you'll easily have enough credits and skills to do this.

Gun Fu
UncommonKill or incapacitate 3 enemies in quick succession with a revolver or pistol in close combat.
Pretty easy at any point in the game. Clearly a high damage weapon helps, so that not too many hits are needed to each enemy. Headshots help too. Fun.
Gunslinger
RareShoot an enemy grenade in midair with a revolver.
This isn't as hard as it sounds, and can be completed anytime/anywhere in the game. Keep a revolver to hand so you can use it in battles where the enemy has grenades.
Having Sandevistan to slow time can be helpful here, so you have more time to aim.

UncommonComplete all Cyberpsycho Sightings.
It's Elementary
RareComplete all gigs and NCPD Scanner Hustles in Watson.
Judgement Day
LegendaryEliminate three bosses from the "Increased criminal activity" category.
There are three "Increased Criminal Activity" locations in Dogtown, which will be visible on your map. with a green triple-skull icon. Each one contains a boss - eliminate all three for this achievement. None are missable and all can be approached anytime.
The checklist indicates the rough map area in Dogtown for where to find each - but given the icons appear on your map they are not hard to locate, the checklist is just to keep track.

Legend of The Afterlife
CommonReach max Street Cred.
Street Cred is a form of rep/XP, given by completing various activities on the streets; side-quests, random encounters and such. The max level is 50 - which you'll easily reach by mid-late game if you're generally participating in everything going on, which you'll need to be for the other achievements anyway. I.e. no special effort is required here; you'll end up achieving this through natural play.

Right Back At Ya
CommonKill or incapacitate an enemy who threw a grenade at you.
This will come naturally as you play through the game; sooner or later somebody will lob a grenade at you, and you know you're going to kill them...
Rough Landing
RareWhile Berserk cyberware is active, perform a Superhero Landing to kill or incapacitate 2 enemies.
Spin Doctor
RareComplete “Run This Town”.
Stanislavski's Method
CommonUse a dialogue option related to V's life path 10 times.
During conversation with NPCs you'll sometimes see lifepath-specific options (e.g. Corpo) in how to respond. Make sure to pick these when they come up. This is missable because there's only so many opportunities in a playthrough (how many depends on the lifepath and how you play; i.e. what conversations come up).
But so long as you're doing most/all of the sidequests (which you'll need to for the other achievements), and making sure to pick the lifepath option whenever it appears in conversation then it won't be difficult to unlock this. There are more than 10 opportunities - just its technically missable because there is only a finite number.

The APB is Not Enough
LegendaryBecome Dogtown's most wanted criminal.
Reach Wanted Level 5 in Dogtown - this can be done anytime just by becoming a murderous rampaging lunatic and killing civilians and police publicly.
You may want to reload a save afterwards of course, just to wash the blood off your hands...
The Devil
RareHelp Takemura avenge the death of Saburo Arasaka.
You must have previously saved Takemura during the mission "Search and Destroy"

The Quick and the Dead

Install the Sandevistan cyberware for its ability to slow time, and kill people whilst its active. As with some other achievements, this can take some time to get through during normal play - so keep it equipped for a while as you play naturally and try to take every opportunity you can to activate it and pop off a few enemies. Once you've reached 50 and popped it you can then swap out the cyberware for something else.

True Soldier

Just grind this out with any melee weapong.
Two Heads, One Bullet
UncommonKill or incapacitate 2 enemies with the same sniper rifle shot.
This can be done anytime in the game, but is quite tricky to get. We recommend carrying a decent sniper rifle, with the highest damage per shot you can get, throughout the game until you unlock this.
Ideally you want two or more enemies in a group that haven't spotted you yet - where you can get a good angle on them for a headshot on the closest and then into the head or body of the person behind.
It won't always happen, even when it looks like it should - and you tend to only get one chance per group of enemies as they'll become alerted and start moving around after the first shot.
Pick lower level/regular enemies as they are easier to kill. Also consider dropping the difficulty for this achievement to have a better chance of killing (vs only wounding) both the enemies.
V for Vendetta
RareAfter reviving with Second Heart, kill or incapacitate the enemy who killed you within 5 seconds.
