Amnesia: The Dark Descent

Amnesia: The Dark Descent is Frictional's landmark hide-in-the-dark horror, and its 100% is low-difficulty but fiddly - a single main-game run is 6-9 hours, and full completion (with the Justine DLC and a Hard Mode run) lands around 15-25 hours. It's more about not missing things than about skill.
The main trap is the THREE mutually-exclusive endings, all decided in the final Orb Chamber and each locking out the others. Keep a separate save just before the ending (or replay the finale) to get all three: Survivor (knock over the three pillars - Daniel survives),
Sacrifice (do nothing and let Alexander complete the ritual) and
Benefactor (the "good" Agrippa-saved ending - brew Weyer's Tonic, take Agrippa's head, and use it on the portal when it opens).
The biggest chores are Illuminatus (every tinderbox - many are level-locked with no going back, so use a checklist) and
Master Archivist (every note across both the main game and Justine). The Justine DLC adds its own mutually-exclusive trio -
Altruist,
Egotist and
Vacillator - needing separate playthroughs. Recommended run: play the main game once with a tinderbox/note guide open, branch-save the finale for all three endings, do a Hard Mode run for
Masochist, then clear Justine's outcomes.
Insanity caps it all off.
Alchemist
RareRemoved the Shadow's residue in the entrance hall in Amnesia: The Dark Descent.
Story-forced. You mix a jar of acid from ingredients you gather, then pour it on the organic Shadow-flesh barrier blocking the way forward near the wine cellar/refinery. Required to progress, so it unlocks naturally.
Altruist
LegendaryNot a single victim died in Amnesia: Justine.
In the Justine DLC, save all three trapped victims (Alois, Basile and Malo) in one playthrough without any of them dying. Mutually exclusive with Egotist and
Vacillator within a single run, so it needs its own playthrough.
Benefactor
LegendaryRescued Agrippa in Amnesia: The Dark Descent.
The "good" ending, and one of the three mutually-exclusive finales (keep a pre-finale save). Earlier in the game you must brew Weyer's Tonic and use it on Agrippa to take his still-living head; carry it all the way to the finale and use it on the portal when it opens during Alexander's monologue. Mutually exclusive with Survivor and
Sacrifice.
Descendant
RareTaken a rough ride on an elevator in Amnesia: The Dark Descent.
The scripted elevator crash - the lift you ride breaks and plunges you down to the Prison. It's on the main path, so it can't be missed.
Egotist
LegendaryNot a single victim survived in Amnesia: Justine.
In Justine, let all three victims die during one playthrough. Mutually exclusive with Altruist and
Vacillator - needs its own run.
Escape Artist
RareEscaped the creature in the water in Amnesia: The Dark Descent.
Survive the flooded Cellar Archives - cross via crates and boxes to stay out of the water, distract the invisible water-lurker (the Kaernk) with thrown chunks of meat, and reach the exit. Required to move on.
Illuminatus
LegendaryPicked up every tinderbox in Amnesia: The Dark Descent.
Collect every tinderbox in the main game. Many are in areas you can't return to once you've moved on, so follow a per-level tinderbox checklist as you go - a missed one means replaying that level. Hence missable.

Unlock all 17 other achievements and this one pops automatically - the last tick.
Masochist
LegendaryMy name is... Daniel... and I... have made life... exceptionally hard for myself.
Complete the main game on Hard Mode (added in a later patch). Hard Mode turns off manual saving, drains sanity and health faster and makes the monsters deadlier - effectively a careful full replay. Best done once you know the game.
Master Archivist
LegendaryRead all notes in all games.
Pick up and read every note - documents and diary entries - across BOTH the main game and the Justine DLC. Some are easy to walk past, so keep a note checklist per level.
NOPE
UncommonLeft when things were getting interesting in Amnesia: The Dark Descent.
A joke one. Right at the start, after Daniel wakes in the corridor, just quit to the main menu instead of carrying on. That's it.
Pipeworker
RareLowered the water level in the cistern to reveal a new way out in Amnesia: The Dark Descent.
In the Cistern, work the mechanism to drain the water and open the route onward to the sewers. Main-path progression, so unmissable.
Quitter
LegendaryFailed to escape the cells and was consumed by the Shadow in Amnesia: The Dark Descent.
A deliberate-death achievement. After you break out of your cell with the Shadow (the collapsing red corruption) chasing you, stop or turn back into it and let it consume you rather than escaping. Save first if you don't want to lose progress.
Restorer
LegendaryCollected the pieces of the orb and rebuilt it in Amnesia: The Dark Descent.
Gather the scattered orb fragments over the course of the game and rebuild the Orb on its stand near the end. Required to reach the finale.
Sacrifice
LegendaryAllowed Alexander to escape in Amnesia: The Dark Descent.
One of the three endings (keep a pre-finale save). In the Orb Chamber, do nothing - don't touch the pillars and don't use Agrippa's head. Wait it out; Alexander completes the ritual and escapes, and the Shadow takes Daniel. Mutually exclusive with Survivor and
Benefactor.
Still Alive
LegendaryBecame aware of the merits of A.S. Inc.'s Span-space Device in Amnesia: Justine.
In the Justine DLC, find and read the specific hidden note about the "Span-space Device", tucked away behind a book on a shelf in the library area.
Survivor
LegendaryKilled Alexander and escaped the castle in Amnesia: The Dark Descent.
One of the three mutually-exclusive endings - keep a save right before the finale so you can get all three. In the Orb Chamber, knock over all three pillars before the portal fully opens; this wrecks the ritual, the Shadow drags Alexander away, and Daniel survives. Mutually exclusive with Sacrifice and
Benefactor.
