Going Medieval's 100% is mostly a natural one - keep a single settlement alive and thriving for a good long while and the bulk of these 94 just tick over: surviving each season (The First Day to
All Year Round), building out your rooms, climbing the research tree, and racking up the hunt/fish/harvest/nutrition counters. A season is 12 days, a year is 48.
The one real trap is the six Grand Objectives (the 1.0 endgame): they are mutually exclusive. Accept one and you are locked out of the rest, so you can only finish one per settlement. Market Mastery,

The Art of War and
No Rest for the Wicked each need their own save - so save before you accept any of them.
Plan your map too: several hunting and taming cheevos are biome-gated. Hares only spawn on Valley/Hills/Mountain maps (not Marsh), and they - with pheasants and rats - get eaten by foxes and polecats the moment they appear, so cull predators early. Mallards and water voles want water/wetland terrain. Start on a varied Valley or Hills map and you will see everything.
Absolute Villainy
LegendaryReach 100% Infamy
Infamy is a hidden settlement stat that climbs as you do genuinely bad things; the confirmed driver is killing innocent (non-hostile) NPCs. Push it to 100% and this unlocks - which is also the gate that lets you accept the Tyrant Overlord objective for No Rest for the Wicked. Best done deliberately on a "dark" save you don't mind burning. Pairs with
Memento Mori.

Hares only spawn on Valley/Hills/Mountain maps (not Marsh), and at 15 HP foxes and polecats instakill them the moment they appear, so people go 100+ hours never seeing one. A Grand Objective (1.0 endgame). These are mutually exclusive - once you accept one you are locked out of the other five, so save first and expect to finish this on its own settlement (see Mind Over Matter,
Natural Order,
Redemption,
The Art of War,
No Rest for the Wicked).
Charter Fair is the trade path: build out your merchant economy (merchant stalls, a market cross, a treasury), level a Broker and barter hard to raise your trading standing. Once it is high enough a broker offers a major trade deal - accept it and send caravans to the marked region-map location. It concludes with a group of merchants arriving at your settlement.
Memento Mori
LegendaryProduce Human Trophy
Get a humanoid corpse (a dead raider does nicely), then butcher it into a trophy: place a Butchering Table, edit its bill to produce a Trophy from the humanoid corpse, and put up an empty Trophy Rack on a wall for it to display on. Grim and quick.
Mind Over Matter
LegendaryComplete New University
A Grand Objective (1.0 endgame) - mutually exclusive with the other five, so save before accepting.
New University is the scholarly path: raise your intellectual standing by levelling a Librarian and running study groups, get a Grand Research Desk into a proper Fellows' Library, and have your scholars write Influential Treatises. Complete the chain to convert the settlement into a university.

Hunt 20 pheasants in total - the 5-kill version is Fowl Play. Predators eat them fast, so keep foxes and polecats down.
Natural Order
LegendaryComplete Pagan Sanctuary
A Grand Objective (1.0 endgame) - mutually exclusive, so save first. This is the Oak Brethren (pagan) path; pursuing it pushes Church-of-Restitution settlers to leave.
Raise your Oak Brethren standing with a Druid and rituals. A pilgrim event reveals a dying mighty oak - retrieve the sapling, plant it in an Oak Brethren Sanctuary, and draw 5 pagan pilgrim witnesses to consecrate it.
No Rest for the Wicked
LegendaryHidden, and the villain ending. A Grand Objective (1.0 endgame) - mutually exclusive with the other five, so do it on its own save.
You first have to reach 100% Infamy for Absolute Villainy - that is what unlocks the Tyrant Overlord objective to accept. Completing the objective is what earns this. The exact steps aren't documented publicly yet (the devs only say "survive the consequences of your actions"), so follow the in-game objective log; I'll fill in the specifics once they're confirmed rather than guess.
