Kingdom Come: Deliverance II

A monstrous masterpiece of an RPG, and a most excellent sequel to the original Kingdom Come: Deliverance.
It has a similar set of achievements; and typically quite a few are both complex and missable.
You can save, and there are saves at the beginning of each chapter as well, but be careful and try to get each achievement as the opportunity arises to avoid having to replay/save-scum too much.
The first few hours of the game are a guided process, until the open world fully opens up as you start the 5th main quest ("Wedding Crashers"). Up until this point there's not too much to worry about except beating Capon in practise for Déjà Vu.
Through your whole playthrough, don't kill or eat any animals for Lent and make sure to do every side quest as soon as they become available for
Been There Done That. These are the big ones that could cost a whole playthrough if missed.
Then, most of the missables either need completion in a specific portion of the story - or need to be completed before the story progresses on too far.
Below are the main points, in main quest order, where there are missables to deal with. Keep an eye on your story mission progress and make sure to tick off each one as you progress:

This Won't End Well
CommonStory. Unlocks automatically during Main Quest 4, Laboratores.
Déjà Vu
CommonYou defeated Capon during practice. Again.
This happens very early in the game, when you first make camp. Capon will invite you to practise in the small fenced off area, and he'll remind you of the moves/techniques. Then you have a practise battle.
You must beat him - if you don't then there won't be another opportunity. You won't be able to manually save at this point in the story - but the game will have autosaved for you. Make sure to use the techniques he just taught you, especially riposte, and if you fail then reload the save. You can't feed your dog afterwards if you skip that bit, but it doesn't matter.
An Old Friend
CommonYou found Pebbles.
Pebbles is your horse, and it gets taken from you early in the game. You can retrieve it anytime after reaching open world gameplay (5th main quest). Pebbles is South-West of Troskoitz, in Semine - at the horse trader. Interact with Pebbles first, then talk with the trader to get it back.
Don't steal your horse back!
Fancy Fighting
CommonYou killed an enemy with a combo.
A combo is a strike where you get multiple hits on the enemy, with melee weapons only. Each weapon is different, and there are lots of different combos which can be pretty effective. The easiest is a straight left/right/left when attacking.
But, this is easily done - even by accident with no idea what you're doing - in the first battle in the intro sequence, just hack away at the enemies attacking the castle and you'll likely unlock it just by chance sooner or later.
Man's Best Friend
CommonComplete the side quest "Mutt", which can be started near Bozhena's Meadow after you've reached the open world by reaching main quest 5 ("Wedding Crashers").
Under Pressure
CommonYou won three skirmishes while bleeding.
This can be progressed anytime throughout your playthrough or after the story. You'll end up bleeding in combat soon enough anyway (or, if you're that good, just let the enemy get a few hits in until you start bleeding).
A red blood icon will be near your health bar when you're bleeding.
Then continue to win the battle. You can heal during combat, but not with bandages as they will stop any bleeding.
A Bird in the Hand
CommonStory. Unlocks automatically during Main Quest 6, For Whom the Bells Toll.
Vengeance
CommonStory. Unlocks in Main Quest 12, Storm (dealing with Istvan Toth). Marks the end of the Trosky act.
New Horizons
CommonStory. Unlocks on arriving in the Kuttenberg region at the end of Main Quest 12, Storm - the transition into Act 2 and the second map.
A Knight's Word
CommonYou rewarded the herbwoman for her help as promised.
In the 3rd main quest ("Fortuna"), in the woods a woman will help you recover from your injuries.
Go back to her later in the game (anytime after opening the open world) and repay your debt of 150 Groschen.
Jack of All Trades
CommonYou have successfully completed every mini-game.
Alchemy and Blacksmithing will be done automatically through completion of main story quests 3 and 6 respectively. The rest you can do anytime through the game.
Rock and Stone!
CommonYou killed an enemy with a boulder dropped from the battlements. Karl would be proud!
Drop a boulder from the battlements onto an enemy through the gate-hole (don't use a bow). Two windows: MQ11 The Finger of God, or the later Kuttenberg siege (MQ27/28 Besieged) - the siege is the easier and final chance.
Nimble Fingers
CommonLockpick a lock that is of "Very Hard" difficulty. You'll need a high Thievery level for this, and you can also use side skills (e.g. Mischief Artist) or potions (e.g. Strong Quickfinger) to make it easier.
The Dirty Seven
CommonStory. Unlocks during Main Quest 15, The Devil's Pack.
Groschen Must Flow
CommonYou spent ten thousand groschen.
This is to spend 10,000 in total across your whole playthrough. Buy anything/everything to get the total up - once you've made some money of course.
You can re-sell bought items, and use whatever cash you get back from that to buy more things; this still counts to increase your total spent to get to 10,000 total. (i.e. you can achieve this with a bit less than 10k, just by re-selling and re-buying - at the cost of quite a lot of lost cash and stuff, of course).
Lights Out
CommonYou slit the throat of an enemy who was looking for you.
This is easiest with guards, at night. Hide (crouch) somewhere near some guards and make a noise by tossing a stone or similar.
You'll know when the guards are then looking for you because there'll be a small white hare icon at the top of the screen.
When a guard comes close, sneak up behind them and do a stealth kill - the achievement will then pop.
Henry the Bruiser
CommonYou beat the best brawler around.
From the 5th main quest ("Wedding Crashers") onwards, you can access the side quest line for this; starting with "Melee at the Mill", which you can begin at Lower Semine Mill. You'll also then need to complete the subsequent side quests "Wine, Women and Blood" and then "The Best for Last".
After you beat the last one, the achievement will unlock.
Reforged
CommonThis can be done anytime after main quest 12 ("Storm"), when you get Radzig Kobyla's longsword. You'll then need 3 Toledo Steel to upgrade it at a blacksmith's.
Without Protection
CommonThis is to break an enemy's shield - for which you'll want a weapon with high blunt damage; like a War Hammer.
Then, you'll want an enemy with a shield (who knew!). Keep bashing at the shield until it breaks - whilst trying not to actually kill the enemy.
Tales of Kuttenberg
CommonYou helped Rosa finish her book.
For this you must complete the side quest "Rosa's Book" during the 18th main quest ("Taking French Leave") - there will be no opportunity after this.
Die Another Day
CommonDuring the 19th main quest an optional objective will appear to protect your brother, Christopher Oderin. Make sure he doesn't die! The achievement will then unlock at the end of the quest.
Shiny
CommonYou won a gold badge in a game of dice.
You have to already have a gold badge for this; so that you can play for winning/losing a gold badge. They can be bought from various innkeepers around the game, and it doesn't matter what kind of badge.
Once you have one, approach a dice table and select to play with a badge. You'll need 300 Groschen to buy in to the game. Not all opponents are able/willing to play for a badge, so if you can't find somebody then just keep coming back to try again (or skip time).
You must play at Emperor level to unlock the achievement - and it helps to have better dice for this to make the game easier. You may come across some on your travels, or you can buy from innkeepers, or steal them from other dice players!
Martyr
CommonStory. Unlocks during Main Quest 24, Oratores - which is also the Kuttenberg point-of-no-return, so wrap up any Kuttenberg side content before completing it.
The End
UncommonStory. Unlocks on finishing the campaign at Main Quest 32, Judgement Day.
Penitent One
UncommonYou made the penitential pilgrimage.
Talk to a priest (Father Nicodemus at Trosky, or Father Marian in Old Kutna) and take the Pilgrimage task; travel to the shrine, pray, and let the roughly two-minute prayer run to completion without skipping.
Silence is Golden
UncommonMake the choices that stop the raid on Semine from happening - this hinges on a dialogue choice in Main Quest 7 as well as the torture/reporting decisions in MQ8 Necessary Evil. Locks at the Trosky point-of-no-return (completing MQ8).
Old Raven
UncommonDuring Main Quest 7, Back in the Saddle, find and free the imprisoned true Lord of Nebakov. Locks once you leave the Trosky region.
Face to Face
UncommonDuring Main Quest 30, Reckoning, find Markvart in the tavern.
No Good Deed
UncommonIn Main Quest 23, Dancing with the Devil, duel and beat Dry Devil first, then open the gates without being detected.
Truce
UncommonComplete either the "Frogs" or "Mice" side quests, then you can get and complete the side quest "Battle of the Frogs and Mice" - BUT whilst doing it, go straight to Bailiff Thrush and convince him to intervene instead.
True Cardinal
UncommonMake the specific dialogue choices at the church council during Main Quest 25, The Italian Job.
The Lord Taketh Away
UncommonYou've lost ten thousand groschen.
A cumulative money-lost counter. Commit crimes and pay fines to guards (or otherwise lose groschen); pay up then reload to recover the funds. The counter may pop below the stated 10,000, so it can come earlier than expected.
Been There Done That
RareFor this you must complete 95% of all quests. When side quests appear, give them attention immediately in case they auto-fail or disappear. Don't fail any quests - reload a save if you do.
Also don't kill anyone who's not hostile - in case they could later be involved in a quest.
My Shoes!
RareSomebody has to steal your shoes whilst you are drunk.
This is easiest if shoes are the only thing you're wearing (i.e. take everything else off) - and then get drunk. Really drunk.
There's some RNG here - you won't always have something stolen whilst blacked out. So you might have to get drunk again the next night to try again!
Master of Masters
RareThis is in the side quest "Lost Honour" in the Kuttenberg region - beat the master swordsman Zavish three times. Available after MQ14; locks at the Kuttenberg point-of-no-return (MQ24 Oratores).
Under a Brown Flag
RareComplete the Kuttenberg side chain "Yackers 'n' Fash" then "Striped Tonies" (available after MQ14). Locks at the Kuttenberg point-of-no-return, MQ24 Oratores.
Master of the Blacksmiths’ Guild
RareYou passed the Trial and became a Master blacksmith.
(Legacy of the Forge) Complete the DLC's first quest, "Adept" - pass the guild trial by answering three randomised blacksmithing questions correctly, then forge a high-quality sword.
Bohemian Sniper
RareYou killed an enemy with a crossbow shot to the head at long range.
Get a crossbow and bolts (loot or buy from a Kuttenberg-region huntsman), find hostile bandits (not civilians) and land a headshot from around 25-30 m. Respawning night bandit camps give reliable targets.
The Hydra's Heads
RareIn Main Quest 22, The Lion's Den, kill all three commanders and collect the evidence.
Weapon of Choice
RareYou've defeated an enemy with every type of weapon.
Kill at least one enemy with each weapon category (short and long sword, axe, mace, war hammer, polearm, dagger, bow, crossbow, pistole, unarmed and the rest - about 20 types). Buy any missing types from Kuttenberg smiths and finish off weakened enemies with each. Regular civilians count too.
Canny Shooter
RareYou won the miners' archery competition without missing the target!
Win an archery contest without missing a shot you take. Save first, drink Archery / Agility potions, and out-score your opponent. (Sources disagree on the exact contest - some point to the Devil's Den event in the Kuttenberg region rather than a literal "miners" one.)
Poet's Gut
RareEvery strophe is a work of art.
(Brushes with Death) During the side quest "Source of Inspiration", when reciting poetry to the old woman Kvyetoslava, pass all four skill checks in a row and pick only the skill-check dialogue options - any failure or non-skill line voids it. Needs around 20 Scholarship and 19 Charisma (the Ladies' Man perk gives +2).
Home Sweet Home!
RareYou completed the reconstruction of your smithy.
(Legacy of the Forge) In the first DLC quest, buy and repair every upgrade slot for your Kuttenberg forge - the room, house and Garden property. Needs high Prestige and roughly 40,000-50,000 groschen in total.
Boneworks
RareYou helped the executioner, knacker, and ossuary guardian.
Complete three side quests: "Forbidden Fruit" (the knacker Ignatius, Trosky cemetery), "Thou art but dust…" (Brother Morticius, Sedletz ossuary) and "The Executioner's Pride" (the executioner Heinz, near Kuttenberg's southern wall).
Martin’s Legacy
RareYou’ve completed the restoration of the Kuttenberg astronomical clock.
(Legacy of the Forge) Complete the DLC quest chain (Martin's Dream, Old Plans... through to "The Last Step"). Finishing "The Last Step", which restores the town-hall astronomical clock, unlocks it.
Warlock’s Riddle
RareYou cracked the riddle of the lost alchemist.
(Legacy of the Forge) During the "In the Service of the Guild - Warlock's Riddle" investigation, assigned by Bushek the Elder, solve the lost alchemist's riddle.
Confessor
RareThe painter told you everything.
(Brushes with Death) Across the quests "Man Proposes" and "God Disposes", exhaust every dialogue option about Master Voyta's past. Follow the specific "Man Proposes" path so all confession options unlock in the final "God Disposes" conversation; you'll need to win the timed speech checks.
Masterpiece
RareYou finished Brushes with Death.
(Brushes with Death) Awarded automatically for completing the final DLC quest "Unveiling", whatever the outcome.
Painted
LegendaryBut not paid for.
(Brushes with Death) During "Stained Honour", after the bailiff's commission, deface his commissioned painting in his tower room (complete the duel optional-objective route first).
Nevermore
LegendaryYou acquired a clever raven.
(Legacy of the Forge) Buy the "Raven" upgrade for your forge chamber - it needs blacksmith Prestige level 25 and costs around 3,500 groschen.
Nothing But the Best
LegendaryYou gave the painter medicine of the highest quality.
(Brushes with Death) During "Root of Evil", brew a top-tier decoction at high Alchemy (around level 16+) using fresh, optimal ingredients so the potion is the highest quality tier, then give it to Master Voyta.
Original Gunslinger
LegendaryThis is doable mid-late game in Kuttenberg. You'll need a belt that can hold 4 weapons, and then four actual weapons; you can buy a belt from the saddler, and guns from Prokop Eldris in the South-West of Kuttenberg. Don't forget the ammunition!
The achievement will pop once you've equipped all four.
Racing the Leech
LegendaryYou won a race against the guard nicknamed Leech.
(Legacy of the Forge) During the story quest "Martin's Dream", find Standa the Fly near the Cvilín / Horschan mine; he owes a debt to a guard nicknamed Leech. Agree to help, propose a race and beat Leech (a fast horse, dropped armour, an Aesop potion and the Head Start perk all help).
High Noon
LegendaryYou shot a gun-wielding enemy with a gun.
You get a Pistole in MQ11, The Finger of God (end of the Trosky section). Easiest is to shoot the lightly-armoured enemy gunner behind the wooden fence on the far left while defending the fortress walls. Later fallbacks exist (MQ23 Dancing with the Devil; the Mad Alchemist at Rabstein Ruin).
Better Safe Than Sorry
LegendaryYou made yourself a plague mask.
(Mysteria Ecclesiae) Do the optional DLC quest "Prevention" and build the mask before entering the crypt - hand over the ingredients (8 Mint or Sage, 1 Cow Skin, 1 Fragrant Oil, 1 Flax Rope) and you receive the plague outfit and the trophy.
Holy Relic
LegendaryYou found a Holy Relic.
(Mysteria Ecclesiae) Story unlock during the 4th DLC quest "Seek and You Shall Find" - progress to the "Examine Boleslav's grave" objective and inspect the grave; it pops automatically.
Henry the Matchmaker
LegendaryComplete the side quest "Hush, My Darling" - physically carry Black rather than leaving him. Available after MQ14; locks at the Kuttenberg point-of-no-return (MQ24).
Fresh Air
LegendaryYou finished DLC3.
(Mysteria Ecclesiae, the DLC3 pack) Unlocks automatically on completing the final DLC quest "The Time has Come" and finishing the Mysteria Ecclesiae storyline.
Hic-Hic-Boom
LegendaryMaking gunpowder while drunk isn't a very good idea.
Get a gunpowder recipe (from Miller Kreyzl via the side quest Opus Magnum, or buy one in the Kuttenberg region), get Henry to the second drunkenness stage (about five bottles), then deliberately botch the recipe at an Alchemy Bench - the failure triggers a game-over screen and the trophy.
Stop Licking That!
LegendaryYou tasted flour... from the hair of a dead person.
(Mysteria Ecclesiae) During the DLC quest "To Dust You Shan't Return", at "Inspect the recovered bodies", inspect the younger corpse's head then hair, and choose "Taste it" (not "Back"). One-time only.
Remember Remember…
LegendaryComplete the side-quest chain "Feast for the Poor" then "The Thieves' Code" then "The Spark" (available after MQ17 Via Argentum). Locks at the Kuttenberg point-of-no-return (MQ24).
Rest in One Piece
LegendaryThe skull finally reached its final resting place.
(Brushes with Death) In "Unveiling", pick the dialogue that convinces Voyta you care (including a medium Speech check on "No, I do care about you") so he gives you the Painter's Skull; complete Man Proposes / God Disposes. Then take the skull to Voyta's grave (marked between Bylany and Wysoka, south of Miskowitz), dig with a shovel and place it.
Overkill
LegendaryFor this you just have to shoot a rabbit with a gun. You'll get a gun automatically in the 11th main quest ("The Finger of God"). Rabbits can be found in the daytime at deer hunting areas.
Save beforehand because aiming can be a challenge. Practise on larger animals as well.
Note that getting this invalidates Lent, so if you're aiming for that then reload your save after unlocking this trophy so your kills stay at zero - or just wait for this until after you've completed the story and already unlocked
Lent.
Quiet as a Church Mouse
LegendaryThe knights never had to escort you to the infirmary.
(Mysteria Ecclesiae) Complete the whole DLC without ever being caught by the patrolling knights during curfew and escorted to the infirmary - play the monastery-trespass sections in stealth. Being caught and escorted voids it for the run.
Slip the Noose
LegendaryYou managed to talk your way out of an execution.
Get sentenced to execution (commit a serious crime such as murder and get caught), then pass the persuasion check with the guard to be released. Needs high Speech / Charisma (around 28-30) or a lenient guard.
All Fun and Games
LegendaryHopefully now Stihlau understand there are some things you shouldn't joke about.
(Brushes with Death) During "Unveiling", free the carpenter's apprentice Chrudosh (tied to a tree at the bandit camp) without alerting the bandits - ideally at night - and exhaust his dialogue; then in Kuttenberg find Journeyman Stihlau (western alley / carpenter's house, daytime) and tell him you met Chrudosh.
Bohemian Graffitti
LegendaryYou painted every type of shield.
(Brushes with Death) After shield painting unlocks (the painter is by the scribe at Trosky Castle), have each shield type painted - Knight, Heater, Pavese and Round/Cuman. Pattern and colour don't matter; the Cuman/Round shield comes from random encounters or Sigismund's camp, the rest are bought in Kuttenberg.
Flagellum Dei
LegendaryYou scored 150 points in the mounted archery competition.
Unlock mounted archery via "Horses for Courses" (Trosky region, Nomads' Camp - talk to Mikolai / Bibrek the Archer). Points are cumulative across attempts, so just keep competing until you total 150. Added in the free Horse Racing update.
Hardcore Henry II
LegendaryYou finished the story in Hardcore mode.
Finish the campaign in Hardcore Mode with at least three negative perks enabled. Hardcore Mode must be chosen when starting a new game - it can't be switched on for an existing save - so commit to it from the outset. Pick your negative perks (Numbskull, Menacing, Sonambulist and the like) at the start.
Survivor
LegendaryYou listened to all of Brother Vojta's jokes... and survived.
(Mysteria Ecclesiae) During the 2nd DLC quest "Anamnesis", reach Monk Vojtiech (Brother Vojta) behind the locked cellar door and keep selecting "Tell me another joke" until he exhausts every joke.
Vengeance Solves Nothing
Legendary(Brushes with Death) During the DLC quest "Portrait in Red", let Burkhard live - steal his helmet, then pass the Speech check to lie to the painter that he is dead.
The Ragman’s Dream
LegendaryYou helped the Ragman save his dream.
(Legacy of the Forge) Complete the final quest of the Ragman's thieving-contract questline in Kuttenberg: when the icon appears, talk to the Ragman, recover the stolen documents and return them to him.
Curiosity Killed the Cat
LegendaryIf you hadn't poked around, Libor wouldn't be swinging.
(Brushes with Death) During "Man Proposes", reforge the madman Libor's sword from the dugout and pass the speech check to convince him to keep it; leave, wait at least 24 hours, then return to find he has hanged himself.
Bohemians Rise Up!
LegendaryBohemians Rise Up!
(Brushes with Death) Have Master Voyta repaint three Knight Shields with specific Bohemian crests: Barry Indented, Gules, Lion Argent; Per Fess, Azure, Moravian Eagle; and Per Fess, Or, Eagle Sable. Some crests unlock late - Lion Argent after the main quest "Judgement Day", Eagle Sable after the DLC quest "Portrait in Red".
Feline Affection
LegendaryYou repeatedly interacted with a cat. Aren't you worried Mutt will know?
(Mysteria Ecclesiae) Interact with the library cat in Sedletz Monastery multiple times (far back-left corner; the "Kitty kitty…" prompt). Call it during Anamnesis, again after, again during "The Time has Come", and once more after finishing the DLC by paying 300 Groschen to re-enter - about four times total.
Lent
LegendaryDo not eat any meat nor kill any animals. Highly missable as one wrong move and (if you don't spot it and reload a save) then the playthrough invalidates this achievement.
Note that any soup is fine - even if it's red and meaty looking.
Wolves may attack you, but so long as you're unarmed and just knock them unconscious then this is fine.
Keep an eye on validity in your player stats (Stats -> General -> Meat Eaten / Combat / Animals) - all must remain at zero.
Against All Odds
LegendaryYou finished the game in Hardcore mode with all negative perks enabled.
Finish the campaign in Hardcore Mode with all ten negative perks active. Set at new-game start; a single all-perks Hardcore run also unlocks Hardcore Henry II, so do both together.
Silent Partner
LegendaryYou handed over the evidence from the underground, and no one died.
(Legacy of the Forge) Complete the "In the Service of the Guild: Silent Partner" investigation - recover the evidence dagger from the abandoned underground shaft and hand it over, knocking guards unconscious rather than killing them. Killing anyone fails the "no one died" condition.
Two Brothers
LegendaryPart of the Kuttenberg Sharpshooters side questline. Resolve the Koniash / Brada debt peacefully using the letter from Bailiff Plümel - don't pay the debt yourself and don't report Raven. Locks at the Kuttenberg point-of-no-return (MQ24 Oratores).
Voice of the Bell
LegendaryThanks to your generosity, Old Kutna gained a new bell.
(Legacy of the Forge) Donate 2,000 groschen in total via the Old Kutna "Alms" activity (to Father Marian) for bell repair, wait about three days, then return before or at noon and examine the bell to hear it ring.
Innovator
LegendaryYou've acquired all the new weapons and armour from your journeymen.
(Legacy of the Forge) Build the armourer's and foundryman's shelters at your forge, then collect all 8 sketches (Armourer I-V, Caster I-III) from Investigation / Thieving Contract activities. Give them to Standa and Pushko, wait a day or two, then buy the crafted items from their shops.
Do Good to All People
LegendaryYou helped the people of the monastery.
(Mysteria Ecclesiae) Complete the side tasks "Maddening Pain" (give Maddie the anti-inflammatory potion) and "Our Old Bread" (Cellarius's nest items, the crayfish book, catch 8 crayfish, deliver to Denes), and cure Monk Vincent in his locked upstairs cell.
Ostraconophobia
LegendaryYou were nipped to death by crayfish.
(Mysteria Ecclesiae) During "Our Old Bread", first read the crayfish book in the monastery library so the crayfish spawn; reduce your own health to near-zero, then repeatedly pick up crayfish from the front until one pinches you to death.
