In Chapter 8, near the beginning, in your new home, when you follow Hugo down the stairs to Lucas' lab keep going all the way down the stairs and explore the crypt at the bottom. Interact with the tomb in the middle of the room to get the achievement.

In Chapter 5, soon after shooting some corpses down from a tree you'll encounter a soldier begging to be saved.  Grab the torch, cross the opening, place the torch in the nearby holder, then cast Ignifer at the torch near the soldier.  He'll thank you, then it unlocks...

In Chapter 9, when you reach the university, don't take the easy route in through the side door - instead take out the two guards and enter using the main door behind them.

This is just after the big semi-circular open area where you can use a rock to have a suspended crate of rats drop and break to kill some other guards.
